REFERRAL PROCESS
Referrals are also accepted from General Practitioners (GPs), treating doctors in hospitals, and hospital discharge planners. Upon receiving a referral, Orange Health Care’s community nursing team schedules a meeting to discuss unique needs and create a personalized nursing care plan tailored to individual requirements.
ELIGIBILITY AND COSTS
Eligibility for Orange Health Care’s DVA Community Nursing services is based on the type of Card (Gold or White) held and whether the individual resides at home or in an independent living facility.
